As long as the ball is on the table the game is considered to be started. You cannot begin the game without the playing ball. When a team gets a goal you have to restart the game. There are two ways in which you can do that. Firstly, you can slide the ball into the hole. The second way is to place the ball on the middle of the playing field. The right to start the game has the team that lost in the previous game.
If the ball leaves the table the game should be started again. Sometimes, it is very common among especially passionate players, it is possible to strike the ball too strong so that it goes out of the table. In such situation there is need stop the game and to restart it again. However, if the ball in some way, maybe it has bounced back, goes back on the playing table than there is no need to restart the game. The players can continue to play without any delays.
